The Partnership to Align Community Care hosted a webinar event that explored the core functions and capacities of a Community Care Hub, particularly around network recruitment, engagement, & support and operations, and highlighted a resource that outlined the core roles and responsibilities of CCHs and could be used as a guide to assess gaps, strengths, and areas of growth.
Attendees gained a brief overview of the CCH model, background on the evolution of the CCH concept and its value proposition, and perspective from CBO and healthcare leaders highlighting best practices where CCHs have effectively aligned on network recruitment, engagement, and operations to address upstream drivers of health for multiple populations.
