Partnership to Align Community Care stakeholders developed a financial toolkit to support CCHs and communities contemplating establishing CCHs in understanding the financial components of operating a hub. These resources feature insight into the importance of business acumen and creating value for stakeholders, financial modeling as a way of assessing the viability and sustainability of a hub, and tools that support the assessment of the healthcare marketplace and various CCH revenue opportunities.
