Exploring Core Functions of Community Care Hubs: A Look at Contract Administration & Compliance and IT & Security

The Partnership hosted a webinar event that explored the core functions and capacities of a Community Care Hub, particularly around contract administration & compliance and IT & security.

When CBOs organize into a community care delivery system to partner with healthcare organizations with the purpose of implementing sustainable and scalable interventions to address upstream drivers of health, the organization that assumes the lead role within that CBO network is the Community Care Hub (CCH).

The Partnership to Align Community Care hosted a webinar event that explored the core functions and capacities of a Community Care Hub, particularly around contract administration & compliance and IT & security, and highlighted a resource that outlined the core roles and responsibilities of CCHs and could be used as a guide to assess gaps, strengths, and areas of growth.

Attendees gained a brief overview of the CCH model, background on the evolution of the CCH concept and its value proposition, and perspective from CBO and healthcare leaders highlighting best practices where Community Care Hubs have effectively aligned on contract administration, compliance, and information technology/security to address upstream drivers of health for multiple populations.
