Effective Governance for Community Care Hubs

In collaboration with USAging’s Aging and Disability Business Institute, the Camden Coalition, Harvard Law School Center for Health and Policy Innovation, and the Center for Health and Research Transformation, the Partnership published a series of resources that include:

• A Guide to Effective CCH Committees (Advisory and Governance) and Committee Charters discussing committees a CCH may establish for governance and other purposes to harness the knowledge, energy, and wisdom of its stakeholders. The brief also talks about the process of chartering committees to ensure their effectiveness.

• A Guide to CCH Bylaws reviewing the major components of a freestanding CCH’s bylaws and considering key questions that organizers of a CCH will want to consider when drafting bylaws. The brief also includes a bylaw template and examples of CCH bylaws that can be used for inspiration.
  
  • Bylaws Template (doc)
• A governance brief, Network Contracting to Address Health-Related Social Needs: Considerations for Governance, examining the unique questions about governance in network-based models of community care contracting. The brief walks through six considerations for navigating governance issues in a network contracting arrangement.
• The Community Guide to Selecting a Legal and Contracting Structure for a Community Care Hub focused on helping your team analyze and choose the best-fit legal structure that aligns with your hub’s capacities, priorities, relationships, and goals.

The Partnership and Camden Coalition hosted webinar events featuring these resources and panels of CCH leaders sharing their experiences and lessons learned with building out governance models for their hubs.
