The Partnership and Camden Coalition hosted multiple peer learning sessions, Coffee & Contracts: A Community Care Contracting Collaborative, focused on Community Care Hub financial sustainability topics.
The Partnership to Align Community Care and Camden Coalition hosted a monthly peer learning series that provided an opportunity for presenters and participants to dive deeper into specific topics related to Community Care Hub (CCH) financial sustainability, Medicare billing codes, and leading contracting practices between CBOs/CCHs and healthcare organizations to effectively address upstream drivers of health.
These sessions allowed participants to:
- Participate in peer-to-peer conversations about experiences, practices, and lessons in the community care delivery space;
- Learn about and engage with stakeholders and leaders of the Partnership to Align Community Care;
- Find opportunities to support new CCHs and get involved in ongoing national initiatives; and
- Generate new learnings for future resources and leading practices.
