The Partnership to Align Social Care is excited to announce its rebranding to the Partnership to Align Community Care.

This transition reflects our evolving focus to advance and scale sustainable and aligned health and community care delivery systems through cross-sector collaboration and co-designed solutions leveraging community care hubs (CCHs) to promote whole-person health.

For more than four years, we have worked alongside a dedicated network of cross-sector stakeholders to co-design sustainable solutions for Community Care Hubs (CCHs).

Today, we are proud to enter our next chapter with a new name: The Partnership to Align Community Care.

While our mission to scale sustainable community care delivery remains the same, we are refining our focus to become a dedicated learning, sharing, and change collaborative.
