Exploring Reimbursement Pathways for Whole Person Health

The National Association for Community Health Centers (NACHC), Spring Street Exchange, and Partnership to Align Community Care hosted a webinar exploring evolving Medicare reimbursement pathways in a changing market to promote whole-person health.

ACOs, CBOs, and FQHCs are redefining care for dual-eligible and low-income Medicare beneficiaries by leveraging Medicare care coordination services to include chronic care management (CCM), community health integration (CHI), and principal illness navigation (PIN). This webinar detailed opportunities to align clinical and community partnerships to improve outcomes and promote sustainable solutions to achieve whole-person, value-based care with Medicare, Medicare Advantage, and D-SNPs.

CBOs, CCHs, FQHCs, and health care sector stakeholders will be interested in this conversation that featured examples of:

  • FQHCs that have built a Medicare/Duals patient base and share best practices in growing these patient populations;
  • Leveraging CHI and PIN codes to close gaps in care, reduce utilization, improve Star Ratings, and conduct Medicaid Recertification for eligible beneficiaries; and
  • Value-based payment models like ACOs, where CHI/PIN services can be integrated as part of care delivery to sustain and expand these services.

Attendees learned how health care and community care providers can use Medicare payment opportunities to address upstream drivers of health in the Medicare and Duals populations--including for Medicaid Recertification--and align clinical and community care delivery, while building financially sustainable partnerships.
